Merkel cell carcinoma (MCC) is an aggressive cutaneous neoplasm that is increasingly recognized as a distinct clinicopathologic entity. In this study, we present the cases of MCC seen in our department over a five-year period, describe the spectrum of clinical presentation, and highlight the major differences with its histogenetic precursor, Merkel cell polyomavirus (MCV)-associated primary cutaneous MCC. From January 2003 through December 2007, clinical charts and tumor biopsies were reviewed for MCC. Forty-seven patients were included in the study. The median age at presentation was 65.3 years. Seventy-seven percent of the patients presented with a localized disease, whereas 23% presented with a locally advanced disease. MCC predominantly occurred on the head and neck (63%), followed by the upper extremities (15%) and lower extremities (11%). Of the primary tumors, 55% arose in the sun-protected sites and 45% in the sun-exposed sites. Sun-exposed sites were more common among women than men (42% vs. 19%; p = 0.039), whereas sun-protected sites were more common among men than women (29% vs. 16%; p = 0.03). Thirty-nine percent of the tumors were associated with the Merkel cell polyomavirus. None of the patients with the Merkel cell polyomavirus-associated MCC presented with local recurrences (0% vs. 26%; p = 0.001) or had a nodal disease (0% vs. 38%; p = 0.001). Patients with MCV-associated MCC tended to have a lower recurrence rate (p = 0.06). MCC is a rare skin cancer characterized by a relatively low incidence, but a long-term overall survival. Women are more commonly affected than men, and Merkel cell polyomavirus-associated MCC is a distinct clinicopathologic entity. Sun-exposed sites are more common than sun-protected sites, and Merkel cell polyomavirus-associated MCC has a better prognosis.Que sería de Borussia Dortmund sin Marco Reus?
